Abordagem reabilitadora em paciente acometido por “trauma por smarthphone” e portador de bruxismo infantil severo: Relato de caso

Abstract

Trauma to the orofacial region with smartphones during use is an emerging etiological factor for dental, facial, and soft tissue injuries in children, especially with increasingly early access by people to these devices. At the same time, bruxism in childhood comprises a parafunctional activity characterized by the act of grinding or clenching the teeth voluntarily and/or involuntarily, which may cause various damages to the stomatognathic system. There are several causes, with behavioral factors being the most relevant (ABE and SHIMAKAWA, 1966). The present study reported the case of a patient, male, 6 years old, who attended the Pediatric Dentistry clinic of the Federal University of Ceará, Sobral campus, complaining of painful symptoms in the bottom region of the vestibular sulcus corresponding to element 51. On clinical examination, said tooth had a darkened color and a fistula in the symptomatic region. Throughout the anamnesis, it was collected that the condition arose after inadvertently dropping a smartphone on the patient's face, which happened intensely. In addition, there was also severe bruxism evidenced in worn faces in all teeth of both arches, revealing reduced remaining structure which made rehabilitation challenging. As a conduct, endodontic treatment was performed on the symptomatic tooth and in the following consultation, a rehabilitative approach was initiated with indirect restorations in order to restore the function and aesthetics suppressed by the wear and tear related to bruxism. For this purpose, restorations were performed in a silicone model using composite resin for subsequent cementation in order to optimize clinical time and have greater predictability, see use of antagonist model. Therefore, the adopted treatment plan aimed to remedy the pain and restore the vertical dimension through a satisfactory rehabilitation within the parameters of occlusion, functionality and return of self-esteem and aesthetic harmony.
