Abstract: Health technologies are indispensable for the entire health system, however, it has become a challenge to balance the need for a sustainable health system when considering limited economic resources and the population's constant need for new technologies. Health Technology Assessment (HTA) has become a vital tool to help managers in decision making, providing all the technical subsidies about the technology in demand, including scientific evidence on safety, efficacy, effectiveness, cost-effectiveness, and other aspects. This study aimed to evaluate the knowledge of pharmacy students from the Pharmacy course of the Federal University of Ceará about Health Technology Assessment. For this work, an online questionnaire was used to collect the student's academic data, as well as the student's previous knowledge and the approach to the subject in the university. The results obtained revealed the majority participation of final semester students, and most of the participating undergraduate students considered their knowledge reasonable, and pointed out that the theme was rarely or occasionally taught at the institution. The students' poor knowledge was reflected in the questions asked, so that the answers obtained were unsatisfactory. In view of the panorama obtained, the importance of improving knowledge in Health Technology Assessment in the Pharmacy course is highlighted, and it is necessary that students are prepared, as future pharmaceutical professionals, to consider and know how to synthesize clinical, organizational, economic, and patient aspects, whether in the public health system or at the pharmacy counter, in order to maximize health benefits to the assisted population.

Abstract: Healthcare-Related Infections (HAIs) are one of the leading causes of mortality in hospitalized patients. In ICU environments there is a high prevalence of these infections, with Pneumonia being the most deadly HAI. In recent years, the rates of microorganisms resistant to antimicrobials have been growing exponentially, imposing severe restrictions on the options available for the clinical treatment of bacterial infections. Thus, knowledge of the microbiological profile of bacteria isolated from patients in a hospital is necessary, as well as knowledge of the microbial sensitivity profile. The objective of this study was to analyze the microbiological profile of bacteria isolated from patients with healthcare-associated pneumonia in a hospital belonging to a health insurance company. To this end, data were collected from the Hospital Infection Control Committee (HICC) IRAS notifications and the reports of Tracheal Aspirate (TAS) and Bronchoalveolar Lavage (BAL) cultures. Data were compiled and analyses were performed by Excel 2019 software. Variables analyzed were gender, age group, types of healthcare-associated pneumonia, bacterial species, resistance profiles, reason for hospitalization, and outcome. The sample consisted of 72 patients and 80 bacterial isolates, with male gender over 60 years old being the most prevalent patient profile. 70.8% (n= 51) of the reasons for admission were for COVID-19. 92.5% (n= 74) were Mechanical Ventilator Associated Pneumonias (VAP), in which Gram negative microorganisms were the most prevalent cause of SSRIs, with 97.5% (n=78). Acinetobacter baumanii was the species most responsible for pneumonia with 40% prevalence (n=32), in which all isolates were resistant to carbapenems. Pseudomonas aeruginosa 22.6% (n=18) and Klebsiella pneumoniae 20% (n=16), were other bacteria with high prevalence. The main resistance mechanism found was enzymatic, in which all were serine-β-lactamases, with KPC, AmpC and ESBL types being the most prevalent. The median number of days between hospitalization and notification of IRAS was 26 days (8-101). 58.3% (n=42) had death as the outcome. Of the outcomes by death, 61.2% of patients had VAP. In conclusion, with the results found, the detection of the microbiological profile of an assistance unit is necessary to know the most prevalent microorganisms and resistance profiles and to enable the most correct empirical antimicrobial treatment.

Associação dos polimorfismos dos genes das apolipoproteínas A1 e B e o risco cardiovascular em pacientes com COVID-19 em atendimento ambulatorial

Abstract: COVID-19 is a respiratory infection with the involvement of multiple organ systems, most of which are due to the high inflammatory component involved, such as cardiovascular risk (CVR), in which lesions ranging from endothelial lesions to the occurrence of Acute Myocardial Infarction are observed. (IAM). The dosage of Apolipoproteins (Apo) A1 and B as well as the Apo B/Apo A1 ratio are good markers of the balance between pro and antiatherogenic particles, acting in the stratification of the risk of AMI. In these Apo genes, the single nucleotide polymorphisms (SNP) rs670 and rs693 of Apo A1 and B, respectively, can quantitatively and qualitatively compromise translation. In the Apo A1 gene, there is the PMN for the exchange of Cytosine (wild allele) by Thymine (polymorphic allele); in the Apo B gene there is a polymorphism by the substitution of guanine (wild) by Cytosine (polymorphic). The objective of this study was to evaluate CVR in patients with COVID-19 in outpatient diagnostic care and to verify associations with genetic polymorphisms of Apolipoproteins. Descriptive, cross-sectional and observational study with patients treated at the COVID-19 emergency sentinel units of the Fortaleza Municipal Health Department during the second wave of Covid-19 cases in the state. The study had 98 participants (18- 60 years old), with a positive molecular biology test for the SARS-CoV-2 virus. The healthy control group consisted of 156 young students without COVID-19. Anthropometric parameters were recorded; in addition, biological samples were obtained for genotyping using Taqman probes, in addition to the determination of traditional parameters of lipid metabolism and serum levels of Apo A1 and B. For statistical analysis, p < 0.05 was considered. The Covid-19 group showed higher values of Total Cholesterol (TC), Triglycerides (TG), Cholesterol associated with LDL (LDL-c), Non-HDL Cholesterol (CNHDL) and TG/HDL-c Ratio. The individuals were stratified based on the reference interval of each test, with the Covid-19 group having a higher frequency of values outside the reference interval. As for the presence of polymorphisms, it was observed that, for rs670 there was no difference between the groups while for rs693, the Covid-19 group showed a high frequency of the polymorphous allele (A) mainly with heterozygous genotype (GA) compared to the control group which has a higher frequency of the wild-type allele (G). We evaluated whether the presence of polymorphic alleles alone exerted influence on serum lipid parameters; it can be seen that the polymorphic allele T (thymine) of the rs670 indicates lower TG values and, consequently, the TG/HDL-c ratio in the group with COVID-19. As for rs693, the control group had higher HDL-c and Apo A values in the absence of Allele A. As for the concomitant presence of the two polymorphic alleles, the doubly polymorphic individuals in the Covid-19 group had higher HDL-c values and lower TG values. The Apo B/Apo A ratio correlated with changes in serum lipids. Individuals with high CT/HDL-c values are 23.2 times more likely to be classified as moderate to high risk. The simultaneous presence of the polymorphic alleles of Apo A and Apo B decreases to 0.011 times these chances.

Título: Percepção das pessoas com diabetes Mellitus sobre o cuidado farmacêutico na atenção primária à saúde
Autor(es): Rabelo, Maria Luana de Freitas
Abstract: Diabetes mellitus is a metabolic disorder characterized by persistent hyperglycemia, deficiency in insulin production or its action, or in both mechanisms. This study aims to describe the perceptions of the person with diabetes about pharmaceutical care, in which it is assisted in the pharmaceutical service in a primary health care unit. This is an exploratory study, with a qualitative approach with the use of collection techniques, observation and interview with those of the person with diabetes followed in a primary health care unit in the city of Fortaleza-Ceará, held from September to November 2022. The analytical categories seized were: pharmaceutical service (physical space, infrastructure, accessibility, flow of care, service to people and human and technical resources) and; perceptions of people with diabetes (disease itinerary, care, activities developed by the pharmacist, importance of the pharmacist's activities, facilities and difficulties encountered in this service). Five people with diabetes were selected to participate in the study, in which the main results were obtained: all of them recognized the great importance of the pharmacist's work. As for the activities carried out by the pharmacist, these persons were mentioned activities to teach about glycemic selfmonitoring and the completion of maps, the provision of information, the act of calling the user of the service to go get their inputs and the action of checking the exams. Regarding the perceived facilities, the main one was the fact that there is infrastructure, materials and items favorable to the performance of the activities and pharmacists have interpersonal and communication skills. For the difficulties, the lack of encouragement of other professionals for more people to seek the pharmaceutical care service was mentioned, so that there is no integrated work among professionals and another difficulty concerns access to the unit, which makes continuous monitoring difficult in the service. With regard to the users' perception of difficulties encountered in the primary health care unit, the main reason was the lack of medical professionals in insufficient numbers to meet the demand, with no mention of difficulties related to the pharmaceutical service. Given this, it can be concluded that people with diabetes assisted in the pharmaceutical service clearly perceive the activities of the pharmacist and value the importance of this professional's work to help promote their health.
