Autor(es): Aguiar, Otaciana Araújo de
Abstract: This paper presents an autobiographical narrative of a Relational Intervention mediator, aiming to report her experience and the effects of this intervention on her personal and professional growth, as well as on the quality of relationships between caregivers and children. Our objective is to gather elements that promote reflection on narrative as a source and methodological tool for research, contributing to the theoretical and procedural aspects of the use of the researcher's own autobiographical narrative. The methodological approach used is based on the qualitative and interpretative approach, from the perspective of narrative phenomenology, in which the author is both protagonist and character. As a theoretical foundation, we used the concepts of Bertaux (2010). By examining the dynamics and challenges faced, this study offers a comprehensive perspective on the effectiveness of relational intervention, contributing to a deeper and applied understanding in the field of developmental psychology and psychotherapy. Justification: The quality of relationships between caregivers and children is a crucial component for the healthy emotional and psychological development of children. Relational intervention, as developed by Chantal Cyr, based on attachment theory, emerges as an innovative and effective approach to improving these interactions. The need to explore the effects of this approach not only on the caregiver/child relationship, but also on the development of the mediators themselves, is fundamental to expanding knowledge in this field. This study is justified by its ability to provide detailed and practical insights into relational intervention practices, enriching the existing literature with a personal narrative that contextualizes the challenges and benefits of this practice. Through this investigation, we seek to inspire and inform researchers and professionals in the field, promoting greater adoption and refinement of relational intervention techniques.

Abstract: The choice of long-acting contraceptive methods can modulate the composition of the vaginal microbiota, with direct implications for the reproductive health of young women. This observational, cross-sectional, and analytical study retrospectively evaluated 358 liquid-based cervical cytology reports from women aged 16 to 25 years using intrauterine devices: copper IUD, levonorgestrel (LNG) IUDs of 19.5 mg and 52 mg. The microbiological variables found included isolated microorganisms (Gardnerella vaginalis, Candida., Actinomyces, bacilli, and cocci) and polymicrobial patterns suggestive of bacterial vaginosis. Statistical analysis used Fisher's exact test and odds ratio (OR) calculation with a 95% confidence interval. The results revealed that users of the 19.5mg levonorgestrel-releasing IUD had a lower chance of colonization by Gardnerella vaginalis compared to users of the 52mg LNG IUD, and a lower prevalence of a polymicrobial pattern suggestive of bacterial vaginosis compared to the 52mg IUD and copper IUD groups. Regarding the presence of Actinomyces, the copper IUD had a higher prevalence when compared to hormonal devices. No significant differences were observed in the prevalence of Candida. It is concluded that a locally released dose of levonorgestrel exerts a differential effect on the vaginal microbiota, with the 19.5mg levonorgestrel IUD associated with a microbial profile showing fewer dysbiosis taxa than the 52mg levonorgestrel IUD and copper IUD, while the copper IUD presents a risk of Actinomyces colonization. These findings reinforce the need for personalized contraceptive counseling that considers not only contraceptive efficacy, but also the specific impact of each method and its dosage on the vaginal microbiome.

Abstract: Objectives: Severe obstetric perineal injuries (OASIS) are related to a significant negative impact on the quality of life of those who suffer them. Such impacts can occur as a result of a series of symptoms, such as anal incontinence (AI), pain, sexual disorders, among others. The present study evaluated the incidence of OASIS, the prevalence of urinary, intestinal and sexual symptoms, as well as their impact on quality of life, in a population of women treated at a tertiary hospital located in northeastern Brazil. Methods: a cohort study was carried out from July 2022 to January 2024, with women who suffered OASIS during childbirth (group with OASIS), these were compared with the control group (group without OASIS), and those who had previous symptoms of pelvic floor dysfunction (PAD). Specific questionnaires (Pelvic Floor Distress Inventory PFDI – 20, Pelvic Floor Impact Questionnaire - PFIQ, Female Sexual Function Index - FSFI, and Short Form SF – 36) were applied in person, during hospitalization (T0), by the researcher and/or UFC fellows, and virtually (Whatsapp, text message) or via telephone contact, three (T1) and six (T2) months after birth . The database was structured in an Excel® spreadsheet and evaluated for normality distribution using the Kolmogorov Smirnov test. The conclusions were based on p values using the traditional cutoff range of 5%, and the descriptive analysis of the variables was carried out using tables and graphs. Results: the incidence of OASIS in the studied period was 1.2%. Primiparity (p=0.03), epidural analgesia (p=0.041), use of a vacuum extractor (p=0.046), and greater newborn (NB) weight (p=0.002) were associated with a greater risk of OASIS. Urinary symptoms were more prevalent in the group with OASIS only at T1 (p=0.01), as well as urinary incontinence (UI), which was more prevalent only at T1 (p=0.047). Intestinal symptoms were more associated with OASIS at T0 (p=0.008) and T1 (p=0.04), and anal incontinence (AI) showed a prevalence of 2.4% at the end of the study. Pelvic symptoms were similar in both groups. In SF – 36, an improvement in quality of life was observed over time. Both groups were dysfunctional in terms of sexuality, with scores below the cutoff point throughout the study, despite the improvement observed. Pain and excitement were worse in the group with OASIS at T2 (p=0.037 and p=0.013 respectively). In the PFDI-20, intestinal and urinary symptoms and the total score were worse in the group with OASIS at T1 (p=0.014, p=0.005, p=0.006), and at T2 only the total score was worse (p=0.048). Urinary, intestinal and general symptoms had a greater negative impact on the quality of life of the group with OASIS at T2 (p=0.028, p=0.04, p=0.017). Conclusion: risk factors for the emergence of OASIS were identified, as well as the physical and quality of life impacts that such injuries cause to these women. There was an association of OASIS with pelvic pain, UI, and sexual dysfunctions.

Abstract: Newborn Hearing Screening (NHS) aims to identify hearing impairments early, allowing access to information and support necessary to prevent delays in language acquisition and development due to hearing loss. During the COVID-19 pandemic, declared in March 2020 by the World Health Organization, pregnant women were exposed to a viral infection caused by the SARS-CoV-2 virus, belonging to a family of coronaviruses responsible for causing respiratory syndromes. Viral infections, such as cytomegalovirus, rubella, and measles, can damage the structures of the fetus's inner ear and/or induce immune-mediated damage in the host. Since COVID-19 is a viral disease, it was speculated whether it could also affect the inner ear structures and have negative consequences for the hearing health of these babies. The objective of this study was to characterize the findings of neonatal hearing screening performed on newborns and infants during the first two years of the COVID-19 pandemic. A cross-sectional, descriptive, quantitative study was conducted. Participants in this study were 3,655 babies born in Fortaleza (CE), who underwent the hearing screening test at the Early Treatment and Stimulation Center (NUTEP) between January 2020 and December 2022. Data collection was performed by consulting the audiology service database. The data were transferred from Microsoft Excel® to Stata/IC 16.0 software, and Fisher's exact test and chi-square test were applied to analyze the association between risk factors and the aforementioned outcomes. For binary outcomes, logistic regression analysis was performed to estimate the Odds Ratio. Of the 125 newborns whose mothers had COVID-19 during pregnancy, 88.8% did not have a Risk Indicator for Hearing Impairment (RIH) and presented normal test results. Of the 12 babies with COVID-19 during pregnancy and RIH, 58.3% presented normal results. The most prevalent risk factor among the 9 identified in the study was a stay in the Neonatal ICU for more than five days, with a relevance of 7%. The findings of this study indicate that maternal COVID-19 infection during pregnancy, in isolation, did not constitute a significant risk factor for hearing impairments detected in newborn hearing screening, but when combined with other risk factors it may increase the likelihood of hearing impairments detected by the newborn hearing screening test. This study was approved by the Research Ethics Committee (CEP) under approval number 7.681.483 and CAEE number 88698825.2.0000.5050.
